SCHOLARSHIP PROGRAM
GUIDELINES
Max’s Lemonade
Stand is seeking nominations for our scholarship program. The amount of
the scholarship is a one-time $1,000 donation. The scholarship is to be
used toward tuition and/or books, and payment will be made payable directly to
the educational facility.
In order for a student to be eligible for the scholarship,
the applicant must meet and/or provide the following criteria:
Completed application;
Certified birth certificate to show U.S. Citizenship;
Birth
Certificate to establish relationship to deceased parent or paperwork to
establish legal guardianship to deceased parent;
Be
a high school graduate aged 25 or less as of July 1, 2010;
Be
accepted as a full-time enrollee at an accredited two or four year college,
university, trade school or technical school;
Certified
copies of one of the following: high school transcript showing 3.0
minimum GPA for Junior and Senior years, report card
from
1st and 2nd years of college showing 3.0 minimum GPA or
Senior High School transcript and 1st year report card from
college showing 3.0 minimum GPA;
Personal
essay from recipient, between 500 – 2000 words (please see the application for
more information);
Letter
of recommendation (not from a parent or legal guardian) from a counselor,
community leader or personal acquaintance;
A
copy of parent’s or legal guardian’s 2009 Federal Income Tax Form 1040; and if
applicable, applicant’s 1040;
A
certified copy of parent’s or legal guardian’s death certificate (parent or
legal guardian must have died due to breast cancer or complications thereof);
Incomplete
applications or missing requested information will not be considered;
According
to the terms of the scholarship, students shall be selected on the basis of
eligibility, essays, grades and financial need.
Deadlines
Applications
are due on or before October 29, 2010, and scholarship winners
will be chosen by November 30, 2010. Applications are
available online or will be available at Max’s Lemonade Stand, scheduled for October
9 and October 10, 2010. Up to 4 scholarships will be
awarded. All scholarships may not be awarded. All winners
will be notified directly, as well as being mentioned on Max’s Lemonade Stand’s
web site. Number of scholarships offered each year will be determined by
the previous year’s fundraising. If you are not chosen, and you still
qualify, you may apply the following year.
Any/all scholarship
recipient(s) will also be acknowledged at any/all Max’s Lemonade Stand’s 2011 event(s)
